Do you find fulfillment in connecting patients with information that will help them make an informed choice about their treatment plan? As a Patient Educator, you will be responsible transforming dialysis care by specializing in new patient education, enabling patients to make an active and informed modality choice – by teaching and educating patients and teammates on ESRD… modality options including lifestyle benefits, challenges, techniques and alternatives. The educator is responsible for understanding the patient’s values and adjusting the education to meet the needs of the patient, leveraging educational materials.
Educate and inform patients, patient families, and teammates about treatment options and overall dialysis education.
Utilizing standard materials to communicate to patients, patient families, and teammates.
Provide assistance with dialysis transitions as required.
Coordinate and communicate with the clinic interdisciplinary team including but not limited to the in-centers nurses, social worker, dietitian and Home nurses to plan education delivery and transition steps, when appropriate, and to complete the education process.

Demonstrated ability to adapt messaging and education approach to the individual patient’s circumstances and learning styles.
Experienced in teaching the adult learner with demonstrated ability to recognize and customize education to individual learning styles.
Experience in project management to coordinate the patient experience with teammates while systemically tracking patient progress through the education process.
Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required; functional proficiency with DaVita applications including but not limited to SNAPPY, Reggie, Laboratory, required within 90 days of employment or position change.
High School Diploma, Vocational/Technical school certificate degree, Associate’s degree or Apprenticeship.

Bilingual
Minimum of one (1) year’s nephrology experience, home dialysis experience.
